New Patient Registration

Welcome to Bidgerdii Community Health Service. To help us provide you with safe, continuous care, we ask all new patients to complete a New Patient Registration and Release of Medical Records Form.  This form gives us permission to request your medical records from your previous healthcare provider, so we can gain a better understanding of your health history.

You can return the completed form by:

Please allow 2–4 weeks for your records to arrive. Once received, a member of our team will review your information and contact you to arrange your first appointment. If possible, include any relevant health details or documents when submitting your form.

  • Important: While we’re happy to welcome you as a new patient, please be aware that we may not be able to prescribe or continue any regular medications until your medical history has been reviewed. This ensures your care is safe, appropriate, and tailored to your needs.

Telehealth Appointments

For your convenience, we offer telehealth appointments, allowing you to connect with a telehealth doctor from home or another safe location if you’re unable to attend in person.

To find out if it’s suitable for your needs, please speak with our friendly reception team on 1800 224 700.

Recall & Health Reminders

To help you stay on top of your health, we use a recall/reminder system (HotDocs) that alerts our team when you may need follow-up care – such as results from pathology, medical imaging, specialist reports, or other ongoing health matters.

  • If URGENT follow-up is needed, we’ll contact you directly to arrange an appointment and find a time that works best for you.
  • If NON-URGENT follow up is required you will receive a SMS message from HotDocs asking you to schedule an appointment.

Phone Messages

If you need to speak with a member of our clinical team, please leave a message with reception. We’ll return your call as soon as possible.

There may be a short delay, as our team is often busy caring for patients – but we appreciate your patience and understanding.

Cancelling Your Appointment

If you’re unable to attend your scheduled appointment, please let us know at least 24 hours in advanceor as soon as possible – by calling our team on 1800 224 700  This helps us offer your time slot to another community member in need of care.

Missed appointments without notice (no-shows) can delay important healthcare for others in the community.

  • Clients who have multiple no-shows will be required to check in daily for appointment availability, rather than being given advance bookings.

This ensures that all community members have fair and timely access to care.  Your cooperation helps us provide timely support for everyone.

Prescription Repeats

Depending on your medical needs, a quick check-up with your doctor may be required before a repeat prescription can be issued. Please allow enough time when requesting repeats, as prescriptions are not always available on the spot.
