Care Plans

Managing your health, together.
If you’re living with a chronic condition or have complex health needs, our GPs can create a Chronic Condition Management Plan (GPMP) tailored just for you.

This plan is a written guide that outlines the treatments, services, and support you need to stay well.

Your care plan helps you:

  • Stay connected to the right services, including allied health and specialists

  • Get ongoing support and regular check-ins with our team

  • Take control of your health with clear steps and guidance

We’re here to make sure you get the care you need—when you need it.

Immunisations

Protect yourself, your family, and your community.
Immunisations are a safe and important way to stay healthy and prevent the spread of serious illnesses.

Our health team provides a full range of free vaccinations, including those recommended specifically for Aboriginal and Torres Strait Islander people through the National Immunisation Program.

We offer vaccines for:

  • Children

  • Elders

  • People at higher risk of preventable diseases

  • Eligible individuals (including yearly boosters)

Not sure what you or your family need?
Talk to our friendly team, or visit the Department of Health website for more information.

Our Rockhampton Clinic runs a weekly walk-in vaccination clinic throughout the year. Call 1800 224 700 (press #1) to check clinic days and times.

Sexual Health Checks (STI)

Your sexual health is part of your overall wellbeing.
Our Sexual Health Team is here to support you with confidential, non-judgmental care in a culturally safe space—whether you’re seeking advice, testing, treatment, or education.

We offer free sexual health checks, including a quick finger-prick blood test with on-the-spot results.

How often should you get tested?

  • Every 3 months if you have a new partner

  • Every 12 months if your partner hasn’t changed

Regular check-ups are a simple way to look after your health and catch infections early—including syphilis, which is increasing in many communities.

Getting tested helps protect you, your partner, and your community.
