Home Support

Commonwealth Home Support Programme (CHSP) provides in-home care and support for Elders in Fitzroy Region, helping to promote independence and wellbeing.

This program assists with everyday needs, supporting Elders to stay connected to community and continue living safely and comfortably at home.

Home Care & Supports

Domestic Assistance

  • Help with household tasks to support a clean and safe living environment.
  • Yard / garden maintenance

Allied Health Support

  • Access to health services to maintain physical and mental wellbeing.

Community Access

  • Providing access to individual and group activities / events.
  • Assisting with medical appointments, shopping, and other essential outings.

Service Coordination

  • We can help arrange the services recommended in your assessment, whenever possible.
  • Our support team is here to meet the individual needs of each Elder, helping you stay active, engaged, and supported in daily life.

Transport

  • Reliable transportation to facilitate community / individual engagement.

Are You Eligible?

Assessing your Eligibility:

You may be eligible for Home Support (CHSP) if:

  • You are Aboriginal and/or Torres Strait Islander
  • You are aged 50 or older
  • Reside in the Fitzroy Region

In addition: 

  1. Meet Eligibility for the Commonwealth Home Support Program (CHSP)
  2. Approved for support via My Aged Care Assessment

If you meet these criteria, our team will work with you to determine your care needs and connect you with suitable support services.

How to Access the Program

Steps to access Home Support Services:

  1. Contact The Elder Care Support Team: They will assess your needs and eligibility for My Aged Care
  2. Schedule an Assessment: A representative will arrange a home visit to assess your needs and determine what support services may be suitable.
  3. Create a Support Plan: Based on your assessment, a tailored support plan will be developed to identify the services you require.
  4. Receive a Referral: Once your support plan is confirmed, you will receive a referral to Bidgerdii’s Home Support Team.
  5. Confirm Your Support: Our team will reach out to arrange services and coordinate ongoing support to fit your needs.
